What are generics?

Introduction

Zyban (bupropion) is a prescription medication used for smoking cessation. It works by reducing nicotine cravings and withdrawal symptoms by impacting the activity of norepinephrine and dopamine, making it easier to quit smoking. Unlike nicotine replacement therapies, Zyban does not contain nicotine but instead modifies brain chemistry to lessen dependence on cigarettes. In some instances, nicotine replacement therapy may be recommended in addition to Zyban treatment. 

Primary Indications:

  • Smoking Cessation: Zyban helps individuals quit smoking by reducing withdrawal symptoms such as irritability, anxiety, and cravings.

  • Off-Label Uses: While Zyban is primarily used to assist smoking cessation, its active ingredient, bupropion, is also used to treat major depressive disorder (MDD) and seasonal affective disorder (SAD) under the brand name Wellbutrin.

Why Prescribed

Zyban is prescribed for individuals who struggle to quit smoking due to strong nicotine dependence and withdrawal symptoms. Physicians may recommend it for:

  • People who have attempted to quit smoking but relapsed due to cravings.

  • Patients who prefer a non-nicotine-based method for quitting smoking.

  • Individuals who need additional pharmacological support in combination with behavioral therapy.

Depending on preferences, Zyban can be used alongside nicotine replacement therapy (NRT), such as patches or gum, to improve the chances of successfully quitting smoking.

How to Use

Zyban should be taken exactly as prescribed by a healthcare provider to achieve optimal results.

Dosage and Administration:

  • Initial Dose: 150 mg orally once daily for three days.

  • Maintenance Dose: After three days, the dosage may be increased to 150 mg twice daily, with at least 8 hours between doses.

  • Treatment Duration: Patients usually begin taking Zyban while still smoking and set a quit date within the second week of treatment. Zyban is often continued for several months or longer, depending on individual and prescriber discretion. 

  • Combination Therapy: Zyban may be used with nicotine patches or gum for better results. However, patients should not smoke while using nicotine products, as this may lead to excessive nicotine exposure.

  • Swallow tablets whole—do not crush, chew, or split.

Missed Dose:

  • Skip the missed dose if the next dose is soon.

  • Do not take extra medication to compensate for a missed dose.

Overdose Management:

  • Overdose symptoms include muscle stiffness, hallucinations, irregular heartbeat, shallow breathing, or fainting.

  • If an overdose is suspected, seek emergency medical care immediately or call Poison Control at 1-800-222-1222 in the US or 1-844-POISON-X (1-844-764-7669) in Canada.

Side Effects

Like all medications, Zyban may cause mild to severe side effects. Patients should be monitored regularly, especially at the start of treatment.

Common Side Effects (Typically Mild to Moderate):

  • Insomnia

  • Dry mouth

  • Increased appetite

  • Nausea

  • Skin rash

  • Changes in taste

Some side effects may improve as the body adjusts to the medication. However, if effects persist or worsen, it’s important to inform your healthcare provider. 

Serious Side Effects (Seek Immediate Medical Attention):

  • Seizures: Zyban lowers the seizure threshold, making seizures more likely, particularly in individuals with a history of epilepsy, head trauma, or alcohol/sedative withdrawal.

  • Severe Mood Changes: Increased risk of depression, suicidal thoughts, panic attacks, aggression, or hyperactivity, especially in younger individuals.

  • Manic Episodes: Symptoms include racing thoughts, excessive energy, reckless behavior, or severe insomnia. This is primarily a concern in individuals with an established history of bipolar disorder. 

  • Cardiac Issues: May cause rapid or irregular heartbeat or severe blood pressure spikes.

  • Vision Changes: Blurred vision, tunnel vision, or seeing halos around lights.

These effects are often rare. Patients experiencing any of these severe side effects should discontinue use and seek immediate medical care.

Warnings and Precautions

Zyban is not suitable for everyone and has specific risks that must be considered before starting treatment.

Contraindications:

  • Seizure Disorders: Zyban can trigger seizures and should never be used by individuals with epilepsy or a history of seizures.

  • Eating Disorders: Patients with anorexia nervosa or bulimia are at an increased risk of seizures while taking Zyban.

  • Alcohol or Sedative Withdrawal: Patients who have recently stopped drinking alcohol, benzodiazepines (e.g., Xanax, Valium), or seizure medications are at risk of severe neurological side effects.

  • Use of MAO Inhibitors: Zyban should not be taken within 14 days of using an MAO inhibitor (e.g., phenelzine, tranylcypromine, selegiline) due to the risk of dangerous drug interactions.

  • Use with Wellbutrin: Zyban contains bupropion, which is also found in Wellbutrin (used for depression). Do not take Zyban and Wellbutrin together, as this increases the risk of side effects.

Precautions for Specific Populations:

  • Suicidal Thoughts in Adolescents and Young Adults:

    • Antidepressant medications, including bupropion, have been associated with suicidal thoughts or actions in younger patients.

    • Close monitoring by healthcare providers, family, and caregivers is essential.

  • Pregnancy & Breastfeeding:

    • Effects on pregnancy are unclear—consult a doctor before use.

    • Breastfeeding mothers should consult a physician, as bupropion may be transferred through breast milk.

  • High Blood Pressure Risk:

    • Some individuals may develop high blood pressure, especially when Zyban is combined with nicotine patches.

To reduce the risk of adverse effects and drug interactions:

  • Avoid Alcohol Consumption:

    • Alcohol increases seizure risk, and stopping alcohol use suddenly while on Zyban may worsen withdrawal symptoms.

  • Limit Caffeine & Stimulants:

    • Zyban can increase nervousness and jitteriness.

  • Avoid Other Bupropion Products:

    • Zyban and Wellbutrin both contain bupropion—do not take them together.

  • Be Cautious When Driving or Operating Machinery:

    • Zyban may cause drowsiness, dizziness, or impaired reaction times.
